Welcome to Amigos de las Americas - Chicago North Chapter

                    2007-2008 Application Guidelines

The following items must be brought by you and your parent(s) to the interview:

  1. $50 application fee; check made out to Amigos de las Americas.

  2. Two passport photos (Page 3 of application; these are in addition to passport photos you need to have taken when you apply for your own passport.)

  3. Three copies of the first two pages of passport (If new passport is http://www.amigoslink.org/involve/vol-app-2008.pdf needed, apply in early November, as it takes 5-6 weeks to process.)
  4. Parent Team Job Description (identify the role/s you would like to take on in Parent Teams)
  5. Training Calendar with volunteer's and parent's initials, agreeing to attend each meeting.
  6. Retreat Permission Slip for permission to attend the weekend retreat.

  7. Program Requirements Contract.

  • Top page signed by applicant and parents.

  • Each subsequent page (all nine) initialed by applicant and parents.

  1. Completed 2008 Application (written in pen or typed) and please note the following for each form.                                

bullet Volunteer Data Sheet & Personal History Form – pages 3-6. Answer completely and thoughtfully. Sign.
bullet Letter of Reference Form – Pages 7-8 should be detached and given to your reference. Write your name on the form and include an envelope. Bring sealed envelope with your signature on sealing to the interview.
bullet Consent and Release Agreement  – page 9-11, must be read/initialed and signed by applicant and both parents/guardian.
bullet Medical Release Form -- page 12,  must be read, and, if volunteer will be 18 in Latin America and agrees to these terms, must be signed by applicant
bullet Standards of Personal Conduct and Community Behavior – pages 14-15, must be signed by applicant (parents must also sign if applicant is under 21).
bullet Letter of Medical Disclosure -- page 16, must be signed by applicant and parents/guardian.
bullet Confidential Health Form I – pages 17-18, must be completed and signed by applicant and parents/guardian.
bullet Confidential Health Form II – pages 19-24, must be completed and signed by physician/clinician. COMPLETE with dates for shots and boosters. Plan for appointment to review immunizations.

Financial Assistance – pages 25-26 (optional). In order to be eligible for full scholarship, may be turned in with application, or (for greater privacy) may be detached, and submitted by mail no later than November 18th to:
