2009-2010 Application Guidelines

A.    Compete Online Part 1 of the Application 
    (see enclosed Volunteer Instruction Sheet for Online Application).
B.    Complete the Paper Part 2 of the Application
The 2010 Volunteer Application is divided into sections. For your convenience, most of the
application can be submitted electronically, but there are several pages that we ask you to print
and physically submit with signatures so please read each section’s instructions carefully.

C.   Bring required Application Part 3 (Chapter Items) to the interview:
 1. Completed Paper Portion of Application (see B. above)
 2. $50 application fee, check made out to Amigos de las Americas.
 3.  Parent Job Description (parent identifies role/s they would like to take).
 4.  Training Calendar with volunteer’s and parent’s initials agreeing to attend each meeting.
 5.  Event/Retreat Permission Slip
 6.  Chicago North Chapter Program Requirements Contract.
    •    Volunteer and each parent/guardian sign.
